The University of Victoria Health Coach Program is a telephone-based program for adults of all ages who are living with chronic health conditions.  They receive one-to-one support from a Health Coach, who provides a 30-minute phone call once a week for a period of 3 months.  Coaches offer encouragement, information and self-management strategies that program participants can use to better manage their health and work toward their health goals.  Health Coaches do not offer any clinical advice or treatment.  This is a self-referral program, and there is no cost to participate. Empathetic, understanding, encouraging and non-judgemental volunteers are required. Participants must be telephone and computer savvy and be available to commit to one phone call per week for three months.
